The Patient Centered Medical Home (PCMH) is a model for providing health care in which your treatment is coordinated through your primary care physician. In Patient Centered Medical Home, your primary care doctor is the head of your health care team, which includes other health care providers, and most importantly, you.
The PCMH approach utilizes electronic health records so team members may see "the whole picture," share information, and work together. This assures that you receive the care you need, when and where it is needed and wanted, and in a culturally and linguistically appropriate manner.
Metropolitan Family Health Network's PCMH team includes an internal medicine physician, family practitioners, physician assistants, licensed clinical social worker, and nursing staff, all of whom are genuinely committed to providing you with excellent health care. Our health care management team, outreach department and dedicated administrative staff also work to bridge any gaps that may exist between you and your care providers.
