Field/Outreach Worker: Full-Time
Under the direction of the Chief Operating Officer, with indirect oversight from the Human Resources Manager, the Field/Outreach Worker must distribute materials where the targeted populations congregate or frequent, with the purpose of providing educational and resource information so as to engage potential patients in the various MFHN programs. The Field/Outreach worker will participate in community activities in order to heighten awareness of Metropolitan Family Health Network services, as well as health maintenance and disease prevention.
Applicant should also be able to conduct home visitation, and to provide certain services, such as parenting education, or to support unique patient needs. Conduct educational presentations and workshops on behalf of MFHN. Must possess a thorough understanding of diverse patient populations' needs and be able to correlate these needs with goals of different programs. Will arrange patient transportation whenever necessary to ensure appointments are kept. Help patients build support systems and bridge gaps in their service plans by making appropriate referrals. Respond to cultural and/or social issues while maintaining patients' respect and dignity. Maintain all necessary chart documentation, including — but not limited to — progress notes and needs assessments. Use chart documentation of patients' records for statistical reporting that must be submitted monthly.
Must have Associate’s or Bachelor’s degree in Human Services, possess a valid NJ Driver’s License and have access to a car with corresponding insurance. Minimum of one (1) to three (3) years experience with diverse populations in a Human Service field required. Bilingual in English/Spanish, a plus.
