Sliding-Fee Scale Discounts

As a Federally Qualified Health Center, Metropolitan Family Health Network may be able to help patients who do not have health insurance plans cover a portion of the cost of care based upon their income. The amount of a discount a patient may be eligible for is based upon a sliding-fee scale, which takes into account household income and the number of people in one's family/household.

If you do not have a health insurance plan and are not eligible for Medicare or Medicaid, we ask that you bring the following information and documents with you on a first visit:
